Each client is provided with an online portal account, where electronic documents are available for review and safekeeping.  Click below to access:

Forms:

Once an intake appointment has been scheduled, clients will receive an email with the disclosure statement attached. This must be signed electronically prior to the first session. Any additional forms will be completed during the intake session. Printed copies of all documents are available upon request. 


FEES:

Initial Intake session: (53-60 minutes)  $150

Individual session: (53-60 minutes)   $125

Individual session: (38-45 minutes) $115


At this time, I accept self-payment (cash, credit/debit cards-no checks), and am in network with most Blue Cross Blue Shield, Cigna, Aetna, and United Health Care plans. It is recommended that you contact your insurance provider and inquire specifically about your plan and what services will be reimbursable. There is no guarantee that your claims will be reimbursed. Self-pay options are available to clients who considered “out of network.” 

Please note: I have partnered with an external billing company (Headway). Their staff works as a liaison between insurance companies and clients, and assists with insurance claims and billing. Clients with qualifying insurance plans may be referred for their services. Upon enrollment, clients will be required to establish an online account, and provide a release of information allowing exchange of financial and treatment related information between Headway and myself. Clinical screening forms are also required within this portal as you begin services. Client questions about billing and insurance may be directed to their staff, as I am not directly involved in their financial practices.
